For all appointments and enquiries, please call & click here to submit patient registration form online.
When you come for your appointment, please bring the following
- Referral letter from GP, family physician or other doctor
- Medicare card, DVA card, pension card
- Have your Private Hospital Insurance information with you
- Reports, X-rays, MRI's, CT scans etc and any other relevant information
Financial Information
- Once the appointment is made our friendly clinic staff will provide a fee for the initial consultation
- Fees need to be paid on the day of the consultation
- Payment can be made by CASH/CREDIT CARD (except AMEX)/EFTPOS
- Should you require surgery, a quote will be provided
